Provides all aspects of clinical and administrative duties involved in the genetic counseling sessions with families at risk for or with known fetal anomalies or hereditary disease. · Reviews incoming referrals/records for accuracy and triages patient medical history into the medical record. Advises on appropriate plan of care. · Contacts patients in advance of their appointment to provide telephone counseling and/or arrange consultation in anticipation of their appointment. Collaborates with MFM and referring physicians on appropriate plan of care. · Performs direct patient care in the form of genetic counseling visits with patients and families to include triage, pedigree construction and analysis, patient/family discussion of genetic conditions and contribution of heredity, and review of available testing options. · Prepares test requisitions, authorizations, and follows up with all ordered tests. Documents all result notes with test result interpretation, communicates test results to patients and referring physicians, and coordinates follow-up testing, peer support, or termination services as needed. · Documents all patient interactions in the medical record including testing, consultation note, procedure note and applicable consultation charge. Closes all encounters within 24 hours of interaction. · Collaborates with peer genetic counselors in coordinating the monthly Connecticut Center for Fetal and Neonatal Care and/or Red Chart Communication. · Formulates and advises medical care team for most appropriate specific genetic testing plan for patient. · Interprets results of genetic testing. Liaison to genetic testing laboratories. · Maintains education requirements of certification, and as needed for patient care. · Travel required between Hartford, Glastonbury, Norwich and New Britain locations
