About The Position

The Nurse Practitioner or Physician Assistant in General Cardiology at the University of Minnesota Physicians will provide and coordinate care for adults with cardiac diseases, ensuring continuity of care across various settings including inpatient and outpatient environments. This role involves direct patient care, assessment, management, and education, with a commitment to exemplary patient service and quality improvement.

Requirements

• Graduate of an accredited Physician Assistant or formal Adult Nurse Practitioner training program.
• Eligible or certified through NCCPA, ANCC, or AANP.
• Licensed or eligible to acquire a license and prescriptive authority through the MN Board of Medical Practice (PAs) or MN Board of Nursing (NPs).
• Able to meet threshold criteria/qualifications for credentialing and privileges.

Responsibilities

• Provide and coordinate care for adults with cardiac diseases throughout evaluation and treatment.
• Ensure continuity of care between hospital settings, procedure rooms, and clinics.
• Share responsibility for assessment and management of patient care.
• Write admission and daily notes, orders, and direct discharge planning and follow-up.
• Work to improve the quality of care to decrease length of stay on the inpatient service.
• Obtain patient histories and perform physical and psychological health assessments.
• Provide patient and family education and counseling.
• Manage patient care through appropriate use of available resources.
• Utilize fully integrated Electronic Medical Record (EPIC).
