Functional Medicine Nurse Practitioner / Physician Assistant
About The Position
Relive Health is seeking a clinically strong, patient-focused Nurse Practitioner to join our growing wellness and longevity practice. This role centers on data-driven care: reviewing comprehensive bloodwork, conducting in-depth client consultations, and designing individualized treatment plans focused on hormone optimization, metabolic health, and overall wellness. The Nurse Practitioner will be responsible for prescribing and managing therapies such as bioidentical hormone replacement therapy (HRT) and GLP-1 medications, monitoring patient progress, adjusting protocols based on lab trends and outcomes, and ensuring all care is delivered safely and in compliance with state and federal regulations. This position requires a provider who is comfortable operating in a concierge-style environment, values preventative and optimization-focused medicine, and can communicate complex clinical information clearly to clients. The ideal candidate is detail-oriented, evidence-driven, and motivated to help patients achieve measurable improvements in energy, body composition, cognitive performance, and long-term health.
