Responsibilities

• Conduct 160 in-home visits per month.
• Perform pre-scheduled in-home visits with health plan members.
• Conduct video visits as needed.
• Review medication and medical history during visits.
• Perform physical evaluations and additional diagnostic tests as ordered by health plans.
