About The Position

We are seeking a dedicated Nurse Practitioner (NP) to provide comprehensive assessments and personalized care plans for patients in Skilled Nursing Facilities (SNF) in Snohomish, WA. This full-time contract role focuses on improving patient care quality, reducing hospitalizations, and aligning care with individual goals during short-term stays.

Requirements

• Active NP license in the state of practice, free of any disciplinary actions.
• Minimum 1 year of NP experience.
• Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care.
• Reliable transportation for assessments in nursing homes, assisted living, and home settings.
• DEA certification.

Responsibilities

• Conduct one-time comprehensive assessments, including physical exams, medication reconciliation, medical history, ADLs, nutritional and cognitive assessments, pain evaluation, fall risk assessment, and social determinants of health evaluation.
• Develop and communicate problem lists and recommended treatment plans for each resident.
• Ensure accurate and complete documentation, including ICD10 coding.
• Collaborate effectively with the interdisciplinary care team.
• Facilitate advanced illness and advanced care planning conversations.
