About The Position

Eddy SeniorCare has a rewarding opportunity for a Licensed Practical Nurse (LPN) to work out of our 385 Watervliet Shaker Road, Latham NY location! This is a full time position supporting both our primary clinic and our home based participants. PACE is a unique program on the forefront of trends in healthcare, combining an insurance role (we are the Medicare/Medicaid insurer for our participants) with a comprehensive healthcare provider role (primary care, home health, adult daycare, transportation, all coordinated by an interdisciplinary team). The primary purpose of this job position is to provide skilled nursing care and prescribed treatments to participants according to professional nursing standards and primary care orders. Care is performed at our center as well as in our participant's homes.

Requirements

• Valid NYS LPN License
• Experience or willingness to become proficient in phlebotomy
• Experience or willingness to become proficient in IV administration of fluids and medications within scope of practice

Responsibilities

• Preparing participant visits in the medical clinic
• Drawing labs, administering vaccines & collecting specimens
• Supporting the management of participants in the community including medication delivery and dispenser set up
• Wound care and other treatments as ordered
• Assisting in the oversight of unlicensed staff, ie. home health aides
