About The Position
The Community Hospice has a wonderful opportunity for a FT RN to work as part of our Hospice Care Team caring for patients and families in the homecare and nursing home setting after hours. Schedule is Monday-Friday 12a-8a, with every other weekend rotation. As an RN you will visit patients who are in need of symptom management as well as schedule visits and coordinate with our team to provide end of life support to patients and families. This is an ideal opportunity for someone who enjoys working closely with a team (our teams include Aides, LPNs, medical directors and NPs with specialized expertise in end-of-life care, social workers, chaplains, complementary therapists and volunteers).
