About The Position

As a Home Health LPN at Trinity Health At Home you’ll provide skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse.

Requirements

• Graduate of an accredited or state approved school of practical nursing.
• Licensure as a LPN in the State of Michigan.
• A minimum of one (1) year of experience in an acute care setting; home care experience preferred.
• Phlebotomy and wound care experience required.
• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam.
• Must possess interpersonal skills sufficient to interact effectively with clients and their families.
• Must have current Driver’s license and reliable transportation to and from work site.

Responsibilities

• Provide skilled nursing care to clients in accordance with the plan of care.
• Administer medications as prescribed.
• Interact effectively with clients and their families.
• Document care using the Epic EMR System.
• Collaborate with interdisciplinary caregivers.
