Requirements

• Graduate of an accredited nursing program.
• Active RN license in Delaware.
• Minimum 1 year of nursing experience (Home Care or Acute Care preferred).
• Valid driver’s license and reliable transportation.
• Willingness to work 1 weekend/month and 2 holidays/year (1 summer, 1 winter).

Responsibilities

• Deliver skilled nursing care to patients in their homes, following physician orders and individualized care plans.
• Conduct initial assessments and collaborate with Population Health Case Managers.
• Supervise and support LPNs and Home Health Aides.
• Focus on achieving excellent patient outcomes and satisfaction.
