About The Position

The Director of Social Services at Lexington Regional Health Center is responsible for assessing each patient’s discharge or social needs, which includes financial, emotional, spiritual, and arranging hospital equipment and referrals. The role involves daily rounds with providers and coordination with interdisciplinary staff and families to address patients' needs. The Director provides interventions related to discharge needs and ensures that each observation and inpatient patient receives assistance from a Social Worker unless they decline the service. The position supports patients and families to improve health, wellness, and rehabilitation through education based on initial assessments in collaboration with professional staff. The Director also assists with application referrals for Medicaid, Medicare, Social Security Disability, and Prescription applications, educates patients and families on health-related and social issues, and advocates for patients' rights. The role promotes interdisciplinary collaboration to achieve continuity of care and participates in quality audits and monitoring hospital performance indicators. Additionally, the Director serves as a liaison between the hospital and community resources, develops budgets, manages staff, and provides critical intervention alongside the Mental Health Counseling team as needed.

Requirements

  • Master’s degree in social work or a related human field.
  • Licensed as a Social Worker and/or Mental Health Provider in Nebraska.
  • Certified Master's Social Worker or Licensed Mental Health practitioner preferred.
  • 1+ years of medical social work experience preferred.
  • Basic Life Support certification required within six months of hire.
  • Analytical skills for maintaining Quality Measure within the department.
  • Advanced communication skills for interacting with patients and staff.
  • Ability to maintain confidentiality regarding sensitive information.
  • Ability to complete professional reports and correspondence.
  • Ability to respond to inquiries and complaints from patients and regulatory agencies.
  • Ability to multi-task and pay attention to detail.

Responsibilities

  • Assess each patient’s discharge or social needs including financial, emotional, spiritual, and arranging hospital equipment.
  • Round with providers daily and coordinate with interdisciplinary staff and family for patient needs.
  • Provide interventions related to the patient's discharge needs.
  • Assist with internal referrals for in-person assistance for discharge needs.
  • Support patients and families to improve health, wellness, and rehabilitation through education.
  • Assess social and emotional factors affecting patients and families.
  • Guide application referrals for Medicaid, Medicare, and Social Security Disability.
  • Educate patients, families, and providers on health-related and social issues.
  • Advocate for patients' rights and help them make ethical choices.
  • Promote interdisciplinary collaboration and coordination for continuity of care.
  • Participate in monthly chart audits and monitor hospital's HCAHPS scores.
  • Serve on various committees as assigned.
  • Liaise between the hospital and community resources for financial assistance.
  • Develop and manage budgets for all areas of responsibility.
  • Manage Social Workers on staff.
  • Provide Critical Intervention alongside Mental Health Counseling team as needed.
  • Maintain patient and staff safety using safety tools.
  • Perform all other duties as assigned.
