About The Position

Requirements

• Active Rhode Island Registered Nurse (RN) License
• Active Basic Life Support (BLS) Certification
• Minimum of 1 year of experience in Home Health nursing
• Proficiency in using Oasis and Homecare Homebase systems

Responsibilities

• Conduct comprehensive in-home health assessments and provide skilled nursing care to patients in accordance with their individualized care plans.
• Collaborate with interdisciplinary healthcare teams to develop and implement patient care plans that promote optimal health and well-being.
• Administer medications, treatments, and wound care as prescribed by physicians.
• Utilize Oasis and Homecare Homebase systems to accurately document patient information and maintain up-to-date medical records.
• Maintain effective communication with patients, families, and caregivers to ensure a holistic approach to patient care.
• Participate in regular team meetings, case conferences, and educational sessions.
