About The Position
The Community Hospice has a wonderful opportunity for a Full-Time Chaplain to join our Hospice Home Care Team in the Albany and Rensselaer County areas. Our Chaplains work cooperatively with the interdisciplinary team to provide added support to patients and families. This is an ideal opportunity for someone who enjoys working closely with a team (our teams RNs, LPNs, Aides, medical directors, social workers, chaplains, complementary therapists and volunteers). Workdays are Monday-Friday 8am to 4:30 pm. The Hospice Chaplain assumes overall responsibility for the coordination of spiritual services; provides spiritual counseling and emotional support to patients/families and hospice team members of all denominations; and participates in the development of the patient/family care plan.
