About The Position

The Care Navigator at Rochester Regional Health plays a crucial role in the Integrated Care team, collaborating with Primary Care Providers, Behavioral Health Clinicians, and psychiatric providers to coordinate and support mental health care. This position is responsible for facilitating patient engagement, providing education on mental health and substance abuse disorders, and ensuring appropriate referrals to treatment services.

Requirements

• Minimum of 2 years of experience in Human Services related field, medical related field, client/patient interfacing experience required.
• Associate's degree (AS) required.

Responsibilities

• Support and coordinate behavioral health care with the patient, patient's Primary Care Provider (PCP), BHC, psychiatric provider, and other Integrated Care team members.
• Provide patient education about common mental health and substance abuse disorders and available treatment options.
• Monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
• Provide or facilitate referrals to mental health clinic/substance use disorder treatment programs as clinically indicated.
• Participate in regularly scheduled Integrated Care team meetings and communicate treatment recommendations to team members and outside collaborators as indicated.
• Facilitate patient engagement and follow-up in care.
• Handle confidential information with discernment and sensitivity.
