The Full Service Partnership (FSP) Case Manager at the Institute for Multicultural Counseling and Educational Services (IMCES) is a vital role focused on providing comprehensive mental health services to underserved individuals. This role is deeply intertwined with direct service to clients, as the Case Manager will carry a caseload and be responsible for the overall well-being of the clients they serve. In this position, the Case Manager will oversee the intake process for new clients, ensuring they are promptly connected with the appropriate services and resources. They will develop and implement individualized treatment plans in collaboration with clients, their families, and other treatment team members. Monitoring client progress and adjusting treatment plans as necessary is crucial to ensure optimal outcomes. The Case Manager will also coordinate care with other service providers, including mental health professionals, medical professionals, housing agencies, and social service organizations, ensuring compliance with all relevant regulations and guidelines. Additionally, the Case Manager will be responsible for collecting and maintaining accurate records of client assessments, treatment plans, progress notes, and other documentation.
