About The Position

Miracle Hill Ministries is South Carolina's largest and most comprehensive provider of homeless services. At our core, we exist to serve people experiencing homelessness-physically, emotionally, and spiritually. Summary / Purpose of Position: The Placement Coordinator is responsible for coordinating the placement of children into Miracle Hill Ministries (MHM) foster homes upon referral from the South Carolina Department of Social Services (SCDSS). This role involves reviewing placement referrals, evaluating available foster families, and facilitating placement logistics. The coordinator also supervises on-call staff on a rotating basis with licensing and care coordination supervisors. This is a full-time, in-office role with flexibility for off-site placement visits as needed.

Requirements

  • Bachelor's degree in social work or related field (e.g., counseling, psychology, sociology, criminal justice, nursing, or recreational therapy).
  • Minimum of one year of social work or related experience.
  • Personal or professional experience with the foster care system.
  • Public speaking skills and knowledge of best practices in foster care
  • Must obtain South Carolina Certified Adoption Specialist certification within six months of hire.
  • Must pass a background check including child abuse registry, sex offender registry, and driving record.
  • Strong verbal and written communication skills.
  • Excellent interpersonal and organizational abilities.
  • Ability to work collaboratively in a team environment.
  • Flexible schedule to meet client and program needs.

Responsibilities

  • Receive and document placement referrals from SCDSS.
  • Review referral information and assess placement needs using critical thinking.
  • Evaluate foster family capacity by reviewing home studies and discussing options with staff.
  • Communicate placement needs via MHM's private Facebook page and directly with Care Coordinators (CCs) and Licensing Supervisors (LSs).
  • Collaborate with the Placement Supervisor to recommend and communicate suitable placement options to SCDSS.
  • Contact selected foster families to determine availability and interest.
  • Obtain signed placement agreements before sharing foster family contact information with SCDSS.
  • Conduct placement visits for long-term placements within 24 hours and document in Extended Reach.
  • Upload all signed placement documentation and request supporting documents from SCDSS.
  • Develop safety plans for new placements when appropriate.
  • Complete initial assessments for long-term placements within 21 days.
  • Prepare discharge summaries for short-term placements.
  • Maintain and submit monthly placement data and weekly reports as required.
  • Participate in SCDSS and MHM staffing meetings and respond to placement disruptions.
  • Coordinate all MHM and SCDSS respite placements as needed.
  • Supervise on-call staff every third week in rotation.
  • Participate in professional development trainings and maintain regular contact with placement hubs.
  • Assist with additional foster care department tasks as assigned.
