About The Position

At Forward Service Corporation we're dedicated to empowering individuals to achieve self-sufficiency through our Foodshare Employment & Training (FSET) Family Stabilization Program. As a Case Manager, you'll be at the forefront of this mission, providing essential case management services and support to our program participants. As an FSET Case Manager within our Family Stabilization Program, you'll play a pivotal role in supporting program participants throughout Dane County. From conducting assessments to providing employment counseling and fostering community partnerships, you'll be at the forefront of empowering individuals towards self-sufficiency.

Requirements

  • Associate degree and two or more years of relevant work experience, or equivalent combination of education and experience.
  • Proficiency in strength-based case management.
  • Familiarity with FoodShare Employment & Training and related programs.
  • Ability to inspire participants towards self-advancement.
  • Strong team collaboration skills and effectiveness in diverse environments.
  • Excellent PC skills, including Microsoft Office proficiency.
  • Knowledge of effective job search techniques and training methods.
  • Valid driver’s license and be insurable.

Responsibilities

  • Deliver individual and group services to facilitate personal, educational, and vocational success.
  • Provide comprehensive case management, including Employability Plan development and coordination with external agencies.
  • Maintain accurate data entry into program database and ensure participant program records are up to date.
  • Cultivate employer relations, enhance community awareness, and forge strategic partnerships.
  • Conduct Job Seeker interviews, develop tailored employment plans, and deliver engaging presentations on job readiness techniques.
  • Offer strength-based, motivational counseling to support employment and training goals.
  • Facilitate connections to educational opportunities and training programs.
  • Deliver supportive services and referrals to community resources.
  • Meet participants where they are at, including traveling to participants’ homes.
