Denver Human Services (DHS) is currently seeking a top-notch, highly dedicated Permanency Social Case Worker to join the Child Welfare Services division working with young adults in the Foster Youth in Transition (FYIT) program at DHS. In this role, you would predominantly perform case management duties involving a variety of complex case situations, with a focus on youth aged 18-21 who had prior involvement in child welfare and have opted back in for continued Child Welfare Services through the Foster Youth in Transition Program (FYIT). HB21-1094: The Foster Youth in Transition Program Act establishes a transition program that allows eligible foster youth to voluntarily continue to receive certain child welfare services until the last day of the month in which a youth’s twenty-first birthday occurs, or such greater age of foster care eligibility as required by federal law. Services provided are client-directed and developmentally appropriate as set forth in and agreed to through a Voluntary Services Agreement (VSA) entered into between the youth and county department. Permanency Social caseworkers will collaborate directly with the youth, supporting professionals and community partners to provide child welfare services to the youth that may include but are not limited to: Enrolling in Medicaid Assistance with securing and maintaining appropriate housing. Case management services including: developing a roadmap to success, securing and maintaining employment, obtaining vital documents and records, and accessing information about relatives and siblings while building permanent connections. Engaging the youth with face-to-face contact at least once per month and maintaining the case documentation to meet all requirements as designated in Colorado Children’s Code Volume 7. Perform other related duties as assigned.

Requirements

  • Requirements as set forth in Colorado Department of Human Services Volume 7 and/or Volume 30 regulations.
  • A bachelor’s degree from a higher education institution related to the job responsibilities of child welfare social caseworker.
  • Course work examples can include and are not limited to, the development of human behavior, child development, family intervention techniques, diagnostic measures, counseling, psychology, or therapeutic techniques
  • Professional, internship, or volunteer work experience in a human services related agency, as required by Colorado Department of Human Services Volume 7 and/or Volume 30 regulations
  • Requires a valid Driver’s License at the time of application.
  • Requires post-hire completion of certification requirements to be a social case worker in Child Welfare or Adult Protective Services in accordance with CDHS regulations.
  • Licenses and certifications must be kept current as a condition of employment.

  • Currently or recently working with youth as a certified social caseworker
  • Is able to develop relationships with youth and identify their strengths and opportunities for improvement.
  • Is comfortable working within the juvenile court system and with youth who may have had involvement with the Juvenile Justice System.
  • Expresses the aptitude to allow the youth to lead their case plans and establish their goals.
  • Is resourceful and organized with the ability to follow through with commitments and understand the needs of this population.
