About The Position

Passionate about foster kids and families! The Foster Care Coordinator cares for foster children and families by providing case management, family support, training, and communication.

Requirements

  • Bachelor's degree in Social Work, or related field, and active Ohio Social Work license (LSW) preferred.
  • Two or more years working with children and families preferred. Foster care experience strongly preferred.
  • This position is contingent upon the satisfactory completion of all of the following: background checks (fingerprints required), drug test, reference checks, Department of Justice National sex offender search, Ohio (and other states of residence) Alleged Perpetrator Search, as well as other checks as required by our licensing agency.
  • Candidates for most jobs must be 21 or older to apply.
  • References, background checks, and drug screenings will be completed on all potential candidates.
  • Direct care positions will require a pre-employment physical, as required by our licensing agency.
  • Must be able to commute reliably for all on-site positions.
  • Some roles require a valid Ohio driver's license and an insurable driving record.

Responsibilities

  • TEAM first.
  • Assist with intakes, facilitating placements of foster youth in assigned homes.
  • Conduct regular home visits, meeting the needs of foster youth and family.
  • Provide excellent customer service to foster families, placing agencies, including but not limited to timely documentation, facilitate transportation, etc., as needed.
  • Provide case management services to help youth remain n the home and/or community, according to the needs of the child.
  • Provide and/or coordinate mental health services for foster children and/or family.
  • Participate in team meetings, clinical supervision, peer/case review, training activities, recruitment activities, etc., as needed.
  • Timely completes and submits all required clinical documentation on assigned youth in electronic health record system.
  • Rotate on-call duties, which includes after-hours crisis management for all youth and handling referrals.
