POSITION SUMMARY: The Foster Care Case Manager at Four Rivers provides comprehensive, coordinated case management services to children in foster care and their families. This role supports the delivery of integrated medical, behavioral health, and social services to promote the safety, permanency, and well-being of children while ensuring compliance with FQHC, state, and federal requirements. The Foster Care Case Manager works collaboratively with medical providers, behavioral health clinicians, child welfare agencies, schools, and community partners to address the complex needs of children in foster care. This role involves assessing client needs, developing individualized service plans, connecting clients to community resources, and monitoring progress to ensure positive outcomes. This position would routinely interact with patients to maintain awareness of needs and satisfaction of services provided. This position is responsible for maintaining professional and technical knowledge by attending educational workshops, reviewing professional publications and establishing personal networks. Performance of duties are both self-directed as well as part of an interdisciplinary team. This position must have the ability to take initiative and exercise skilled judgment as well as decision making and problem solving. The Foster Care Case Manager is responsible for overseeing the daily operations of The Landing, the Four Rivers clothing closet and resource hub, to ensure individuals and families in need have access to clean, appropriate, and well-organized clothing and essential items. This role includes inventory management, volunteer coordination, donor relations, and client service to support dignity, efficiency, and community impact. ESSENTIAL Position RESPONSIBILITIES: Conducts comprehensive, and socially sensitive assessments and assists in developing care plans with interdisciplinary health team both within Four Rivers and other partnering organizations. Conducts comprehensive biopsychosocial and social determinants of health (SDOH) assessments. Provides information and education about health/mental health resources to individual patients and to groups. Coordinates medical, dental, behavioral health, and specialty care services. Documents assessment and treatment data in medical record. Provides social work consultation to clinic staff and community providers as appropriate. Serves as liaison between Four Rivers and external agencies, nonprofit organizations and other resources. Utilizes HER, DRVS, Cyber, EMOMED, and outside facilities' EMR systems to obtain patient records for care coordination Manage daily operations of the clothing closet Receive, sort, organize, and inventory donated clothing and items- ensure clothing is clean, safe, seasonally appropriate, and in good condition. Coordinate special initiatives (back-to-school, winter coats, job interview attire, etc.) Prepare basic reports on usage, inventory, and program outcomes. Promote dignity, respect, and confidentiality when serving clients. Other duties as assigned.
