As a Foster Care Case Manager at Health Connect America, you'll play a critical role in supporting children and families within the foster care system—ensuring they receive the coordinated, compassionate care they need to thrive. What You'll Do As a Foster Care Case Manager, you'll oversee a caseload of children and families, ensuring services are well-coordinated, plans are individualized, and every child’s needs are met with consistency and care. Your work will be relationship-driven, detail-oriented, and focused on long-term outcomes. Day to day, you can expect to: You’ll also play a role in supporting foster parent recruitment, assessment, and ongoing engagement—helping build a network of caregivers who are equipped to provide safe and supportive homes. Supporting Quality & Compliance In this role, you’ll ensure all services and documentation meet Health Connect America (HCA), state, and Council on Accreditation (COA) standards. You’ll be expected to maintain high-quality work, meet required timelines, and actively contribute to continuous improvement efforts—ensuring the best possible outcomes for the children and families you serve.
