About The Position
The Forensic Case Manager at St. Mary's General Hospital is responsible for providing personalized, collaborative, and flexible case management and outreach services primarily off-site. This role focuses on engaging, supporting, and integrating individuals with serious mental illness who are involved with the criminal justice system into their chosen communities, facilitating their access to available resources and supports to maximize their independence. The primary goal is to assist consumers in avoiding arrest, reducing incarceration days, or aiding in their reintegration after incarceration.
