The Forensic Care Coordinator Case Manager at Lutheran Social Services (LSS) is responsible for providing community supervision and support to individuals with mental illness who have committed a crime but were found not guilty by reason of mental disease or defect. This role involves case management, assessments, and facilitating access to community resources to support clients' mental health and recovery.   • Perform assessments, investigations, and screenings to determine entry and placement in the Conditional Release Program.
  • Assess client's needs utilizing required assessment tools (e.g., functional screen).
  • Re-assess clients as needed and at least annually.
  • Work alongside clients, their families, and support systems to develop and implement service plans.
  • Facilitate care/service planning and service facilitation.
  • Experience in case management or a related field.
  • Knowledge of mental health diagnoses and treatment options.
  • Ability to perform assessments and develop service plans.
  • Experience working with individuals in the criminal justice system.
  • Familiarity with community resources for mental health support.
