As a Forensic Behavioral Health Navigator, you will play a vital role in helping justice-involved individuals successfully transition and integrate into the community. This position works at the intersection of behavioral health, corrections, and community services — coordinating care, strengthening partnerships, and ensuring individuals receive the right services at the right time. Requirements

  • Bachelor’s degree and professional experience in: education, psychology, social work, sociology, human services, or a related field and experience within a specialized developmental disabilities service system in delivery of habilitation and/or case management services.
  • Ability to travel required.
  • Travel will be based on necessity and could vary from week to week.
  • Potential overnight travel may occur.
  • Specific positions in this class may require the incumbents to possess a valid driver's license and provide a passenger vehicle with adequate liability insurance, or the ability to provide independent authorized transportation, in order to perform work-related travel, such as transportation for clients.

Nice To Haves

  • Master’s degree in social work, criminal justice, psychology, or a related field
  • Professional certification (e.g., Clinical Social Worker, Certified Peer Support Specialist)
  • Experience working with the judicial system and behavioral health system
  • Knowledge of and ability to navigate community resources
  • Experience working with justice-involved mental health populations
  • Proficiency in Microsoft Word, Excel, and Outlook
  • Knowledge, Skills, and Abilities The successful candidate can manage diverse relationships, communicate effectively, stay organized, and take initiative.
  • Strong skills in problem solving, prioritization, goal setting, and conflict resolution
  • Ability to work effectively with individuals who have mental health and other complex needs
  • Ability to collaborate with a wide range of individuals, agencies, and organizations
  • Knowledge of criminal justice and behavioral health systems
  • Knowledge of state laws related to competency to stand trial and insanity proceedings

Responsibilities

  • Coordinate and oversee the delivery of services
  • Conduct assessments and make recommendations
  • Develop plans for services and support
  • Document goals of services and progress towards goals
  • Provide appropriate referrals and follow-up to ensure continuity of care
  • Monitor provision of services
  • Ensure health, welfare, and safety of those we support
