About The Position

The Food-Based Healthcare (FBH) Coordinator will provide direct support to Foodlink’s implementation of the NYS 1115 Medicaid Waiver. Key responsibilities include: inventory and vouchering, coordination of the FoodRx Box service, execution of the Cooking Supplies service in tandem with FBH director, and coordination of the Pantry Stocking/screening services offered in collaboration with our member pantries.

  • Ability to clearly communicate (speak, read and write proficiently) in Spanish a plus.
  • While this position is primarily Monday through Friday during business hours, an ideal candidate would have the ability to work flexible hours on occasion, which may include evenings and/or weekends.

Responsibilities

  • Conduct regular inventory of FoodRx stock on curbside market and order food to keep this at par via Agency Express weekly/as needed. Complete necessary internal documentation as applicable including AP Vouchering, Invoicing etc.
  • Help to receive and stock product for FoodRx on curbside, including solving for growing space needs and navigating box packing obstacles.
  • Coordinate FoodRx warehouse inventory, pars, ordering and substitutions with the Food Bank, including AP vouchering, and navigating potential changes and/or shortages.
  • Coordinate FoodRx Box service with subcontractors from onboarding/scheduling, to payments, invoicing and troubleshooting.
  • Execute Cooking Supplies service in collaboration with Director of FBH, including order fulfillment, invoicing, vouchering and follow-up.
  • Assist FBH team with system (“Findhelp”) management and maintenance to ensure timely calls, scheduling and documentation of services.
  • Conduct regular inventory of Pantry Stocking stock in Food Bank and order food to keep this at par as needed. Complete necessary internal documentation as applicable including AP Vouchering, etc.
  • Coordinate with member pantries to discuss need, delivery, storage and workflow for ordering and receiving pantry stocking shipments to serve these members.
  • Enter all relevant information for this service into Findhelp to track in accordance with FLIPA and DOH.
  • In conjunction with Director of FBH, establish and execute a tracking and monitoring process with member pantries to ensure delivery confirmation of all pantry stocking boxes to receiving member for reimbursement. Complete reimbursement requests in FindHelp and necessary invoicing in CERES.
  • Ensure member care plans are updated in Findhelp as relevant.
  • Works to coordinate supplies, trainings and support between FLIPA, member services and member pantries to ensure screening implementation is successfully started and compliant with FLIPA/DOH/Medicaid standards.
  • Correspond with member pantries regularly, once screenings have started, to assess need, support screening tracking and growth, and answer questions to ensure accuracy and compliance.
  • Using Findhelp, collect names and corresponding screenings from each member pantry to update findhelp and file for reimbursement from FLIPA at least weekly. Create relevant corresponding invoices for FLIPA for this work to track cash flow in accordingly.
  • Other duties as needed with all three parts of this job.
