About The Position

Tabor Hills Senior Living Campus, a not-for-profit community located in Naperville, IL, has been dedicated to caring for our senior population for over 130 years. Tabor Hills Senior Living offers an Independent Living Community, Supportive Living Community and a Healthcare Facility (skilled nursing care, post-acute medical and rehabilitation.) We have a rare opportunity for a full time 2nd Shift Floor Nurse to join our team on our Healthcare Campus! STARTING RATE-$37.00-$38.00 DEPENDING ON EXPEREINCE FULL TIME 2ND SHIFT-3:00 P.M. TO 11:30 P.M. Requirements

  • Valid Illinois LPN License.
  • Current CPR Certification required.
  • Must be capable to execute all terms and conditions set forth in the EMPLOYEE HANDBOOK, including, but not limited to: a. Responsibilities

  • Participate in assessing, planning, implementing and evaluating resident care delivery and actual resident outcome.
  • Completes initial and ongoing nursing assessments needed to plan care interventions.
  • Make frequent rounds to ensure care delivery meets quality care standards and restorative protocols.
  • Utilize appropriate infection control techniques to prevent spread of infection.
  • Administer prescribed medications and treatments in accordance with approved nursing protocols.
  • Monitor vital signs and hydration status to detect deviations from normal limits.
  • Perform direct resident care.
  • Assemble and utilize resident care equipment and ancillary supplies.
  • Obtain specimens for laboratory exam.
  • Take, transcribe and implement physician’s orders.
  • Requisition and arrange for diagnostic and therapeutic services, as ordered by resident’s physician, from departments within the institution or from outside the facility.
  • Order prescribed medications and requisition supplies and equipment from appropriate sources.
  • Describe resident’s responses to treatments/medications, response or lack of progress towards care plan or restorative goal in nursing notes and monthly summary documentation.
  • Provide support to family members in coping, participating or managing residents’ care needs.
  • Assist physicians during rounds and examinations .Notify appropriate departments to initiate physician orders.
  • Participate in the assessment, planning and implementation of nursing staff efforts in providing quality resident care.
  • Participate in discharge planning in coordination with other departments/services.
  • Coordinate activities between departments and other services.
  • Monitor performance of nursing personnel against established standards and provide education to improve care delivery.
  • Assist with orientation, instruction, and supervision of other nursing personnel.
  • Supervision of Resident nutrition and hydration; assist residents who cannot eat independently.
  • Monitor residents’ status or changes in condition and document findings in medical record, and report same to family, immediate supervisor and physician.
  • Participate in planning and conducting training to other staff members.
  • Give report to CNA’s working in your assigned area.
  • Review all physician orders every shift.
  • Communicate with the Director of Nursing on resident matters.
  • Provide direct resident care as necessary.
  • Observe resident and document significant changes in condition, abnormal lab values, response to treatments/meds, and significant incidents, and notify nursing supervisor, physician and family of change, when indicated.
  • Give and receive comprehensive report of residents’ condition.
  • Coordinate telephone inquiries.
  • Periodically review resident care plans with resident, supervisor, resident’s physician, certified nursing assistants, and when appropriate, with the resident’s family.
  • Maintain resident records and perform charting duties.
  • Ensure conformance to established practices related to drug administration.
  • Maintain controlled drug records and report any discrepancies to Supervisor immediately.
  • Attend and participate in scheduled in-service programs and staff meetings.
  • Actively participate in Facility Committee meeting when appointed.
  • Maintain confidentiality of necessary information.
  • Utilize supplies and equipment properly and without waste.
  • Charge supplies per facility policy.
  • Think and act calmly and logically to meet unusual occurrences of the job without being thrown off stride.
  • Report all personal incident/accidents immediately to your Supervisor.
  • Perform any miscellaneous work assignments as may be required.
