RN Case Manager – Floating Territories
About The Position
Home Health Care, Inc. is a leading home care agency in the Twin Cities, committed to providing individualized, compassionate care in a family -centered work environment. As an RN Case Manager, you’ll have the resources and support to make a true impact, offering 1:1 care in a setting that values each employee’s contribution. In this full -time RN Case Manager role, you’ll assess clients’ health needs, provide skilled nursing care, and guide caregiver training across the Twin Cities Metro Area. You’ll experience the satisfaction of delivering quality care directly to clients in their homes, with the support of a collaborative and dedicated team. You will also move around to help with caseload fluctuations across designated territories to ensure a timely initiation of care and progress clients through their care plan.
Requirements
- Minnesota RN license
- Reliable transportation with insurance
- Proficiency in nursing assessments and client communication

- Experience in home health care is a plus
