About The Position

We are seeking a skilled floating Case Manager to support clients by assessing needs, developing stabilization and re-housing plans, and connecting clients to resources. The role requires clinical expertise, experience with homelessness and related challenges, and the ability to work both independently and collaboratively within an interdisciplinary team.

Requirements

  • Bachelor's degree from an accredited school is required.
  • Must have a valid Food Handler’s License, or the willingness and ability to obtain one within six (6) months of hire, is required.
  • Minimum of 3-5 years of experience working shelter or supportive housing.
  • Experience working with homeless/formerly homeless population, mentally ill, substance abuse and/or low-income population.
  • Strong relationship building, written and oral communication skills.
  • Strong Computer skills required including Word and Excel.
  • Ability to work independently as well as a member of an interdisciplinary team.
  • Highly organized and able to manage a diverse caseload of clients with diverse needs.
  • Able to work well under pressure including in crisis situations and able to be flexible with work schedule when necessary.
  • Track record of proactive, strength-based work with individuals and families.
  • Experience with ETO, or other electronic case management systems.
  • Floating Shelter Case Manager MUST have previous experience with CARES

Nice To Haves

  • Knowledge of HIV/AIDS, mental health, substance use or chronically homelessness.
  • Experience working with evidence-based models, specifically Motivational Interviewing.
  • Bilingual a plus.

Responsibilities

  • Conduct thorough client needs assessments to inform individualized service plans.
  • Develop and implement action plans aimed at client stabilization and re-housing.
  • Identify, coordinate, and leverage internal and external resources and services.
  • Monitor, evaluate, and document client progress and interactions.
  • Provide advocacy and support to help clients achieve physical, emotional, vocational, educational, and social goals.
  • Apply best practices within social services to address complex client needs.
  • Collaborate with interdisciplinary team members to enhance service delivery.
