About The Position

The Support Staff III / Floating Case Manager provides flexible, short-term case management coverage across residential treatment programs to ensure continuity of care, timely intakes, and consistent client support. This role assists with intakes, transportation, documentation, and case management functions during staff absences, high census periods, or increased program needs. The Floating Case Manager maintains a small caseload and works in alignment with Nat-Su Healthcare policies and standards.

Requirements

  • High school diploma or equivalent required
  • Valid driver’s license with an acceptable driving record required

Nice To Haves

  • Case management or related experience preferred
  • Experience in behavioral health, human services, or residential treatment settings preferred
  • Experience working with court-involved populations preferred
  • Case Manager certification preferred or willingness to obtain
  • Proficiencywith electronic health records (EHR) systems and Microsoft Office applications
  • Strong interpersonal and communication skills
  • Ability to work effectively with diverse populations
  • Ability to manage competing priorities and adapt to changing program needs
  • Ability to maintain confidentiality and comply with all privacy regulations
  • Ability to work independently and collaboratively within a team

Responsibilities

  • Support residential intakes as assigned
  • Welcome and orient new clients to the program
  • Assistwith intake paperwork not completed by Admissions
  • Complete assigned case management assessments and intake checklists
  • Ensure intake-related tasks are completed within required timelines
  • Provide temporary or supplemental case management coverage as needed
  • Maintaina small caseload based on program needs
  • Assistclients with goal development, engagement, and compliance
  • Monitor adherence to treatment, program, and court requirements
  • Transport clients to court, probation, and community appointments
  • Coordinate appointment scheduling and transportation logistics
  • Communicate outcomes and concerns related to transportation
  • Attend Program Staff Meetings
  • Attend Clinical, Case Management, and AP&P meetings as assigned
  • Communicate updates to leadership and assigned Case Managers
  • Maintain accurate and timely documentation in the Electronic Health Record (EHR)
  • Complete intake, service, and case management notes
  • Report incidents, non-compliance, and concerns per policy
  • Ensure all documentation meets regulatory and agency standards
  • Perform other duties as assigned
