About The Position

The Float Pool (RN) Case Management Lead Analyst role at Evernorth Health Services is a full-time telephonic/digital position focused on managing healthcare needs for clients. The role requires a Registered Nurse (RN) to work collaboratively with clients, families, and healthcare providers to assess medical histories, establish care plans, and ensure quality outcomes. The analyst will be responsible for managing a high volume of calls, documenting case management interventions, and adhering to quality assurance standards while promoting effective communication and customer advocacy.

Requirements

  • Active unrestricted Registered Nurse (RN) license in State of Residence.
  • High-performing candidate with Premium Case Management experience.
  • Excellent time management, organizational, research, analytical, negotiation, and communication skills.
  • Proficient with computer and case management systems.
  • Access to high-speed broadband internet and a private workspace.

Nice To Haves

  • Experience in Pediatric Nursing (PDN).
  • Case Management certification.
  • Experience with TruCare and ICMS systems.
  • Excellent telephonic and digital communication skills.
  • Strong computer skills in Microsoft Office Suite.

Responsibilities

  • Obtain informed verbal and written consent from clients.
  • Identify, support, and resolve customer healthcare needs promptly.
  • Establish collaborative relationships with clients, families, and healthcare providers to assess care needs.
  • Set short-term and long-term goals for case management and involve all relevant parties.
  • Promote quality, cost-effective outcomes through effective communication and health advocacy.
  • Implement, coordinate, monitor, and evaluate the case management plan as needed.
  • Document findings clearly and accurately in the case management system.
  • Act as a liaison between clients, families, and healthcare providers.
  • Maintain accurate records of case management interventions and outcomes.
  • Adhere to quality assurance standards and case management policies.
  • Demonstrate sensitivity to culturally diverse situations and clients.
  • Provide leadership and mentorship to other case management staff as needed.
  • Comply with all accreditation and regulatory mandates.
  • Participate in training initiatives to maintain clinical expertise.
