About The Position
Become a part of our caring community and help us put health first. As a Home Health Registered Nurse, you will provide admission, case management, and follow-up skilled nursing visits for home health patients. You will administer ongoing care and case management for each patient, provide necessary follow-up as directed by the Clinical Manager, and confer with physicians in developing the initial plan of treatment based on physician's orders and initial patient assessment. You will also coordinate appropriate care, encompassing various healthcare personnel, report patient care/condition/progress to the patient's physician and Clinical Manager, and implement patient care plans in conjunction with patients and their families. Additionally, you will oversee the work of Home Health Aides and ensure proper documentation is completed. Discharge patients after consultation with the physician and Clinical Manager, and prepare appropriate medical documentation on all patients.
