About The Position

Physical Therapist PRN- Physical Therapist PRN- Non-benefitted, Contingent Trinity Health Chelsea Hospital Location: Chelsea, MI Schedule: Flexible Scheduling- weekend and holiday rotation Rate: $58.19 Position Summary The Physical Therapist provides evaluation and treatment of patients in a range of specialty programs. The Rehabilitation Services Staff Physical Therapist provides evaluation and treatment of patients in a range of specialty programs. Essential Functions and Responsibilities Demonstrates self-directed learning. Provides quality patient care by considering the age specific, development and cultural needs through competent clinical practice. Demonstrates unit/area competencies. Demonstrates competency with individualized assessment of patients using a wide variety of assessment measures and procedures. Stays up to date on all participating insurance guidelines for rehabilitation services. Provides thorough documentation on each patient interaction. Documentation must be timely and include all pertinent information required by state law, accrediting agencies and 3rd party payers. Formulates individualized, age specific, therapy goals based upon assessment results and patient needs. Consults with other team members and selects the most appropriate plan of care consistent with the needs and capabilities of the patient. Implements patient care in a manner consistent with the therapy goals and the changing needs of the patient, using a variety of therapeutic approaches. Completes daily charges for each patient while adhering to ethical practices per insurance guidelines for each payer. Orients patients and families to the therapeutic purposes and procedures of physical therapy, and educates them about home programs, impairments and progress. Provides re-evaluations/re-assessments of patients on a regular basis to ensure progression towards goals. Supervises Physical Therapy student affiliates as assigned by the Site Leader or Student Program Coordinator. Provides for own professional growth and education and contributes to staff development activities. Participates in regularly scheduled staff meetings and in-services. Other Functions and Responsibilities Performs other duties as assigned. Required Education, Experience and Certification/Licensure Licensed as a Physical Therapist in the State of Michigan. Requirements

  • Licensed as a Physical Therapist in the State of Michigan.
