Provide community hab services as indicated on the Individuals Service Plan and community hab plan. Implement the goals outlined in the Community Hab Staff Action plan. Maintain records of the individuals being served in the community hab program and provide them to the Home & Community Services Supervisor weekly. Observe person we support for any unusual physical or behavioral changes and report to supervisor. Attend and satisfactorily complete all required training as assigned. Demonstrate flexibility, accuracy, and confidentiality in work assignments. Maintain confidentiality of all work related information.
