About The Position

The Application Support Specialist assists individuals in New Hampshire seeking access to Medicaid Long-Term Care (LTC) program with understanding and completing financial eligibility determination and redetermination applications. This role involves delivering hands-on application assistance and educate individuals, families, providers, and case managers about application requirements and federal and state financial regulations at nursing homes, assisted living facilities, hospitals, and other agency-designated locations across New Hampshire By delivering assistance, incumbents serve as key point of contact for applicants and their families to improve understanding of eligibility determination and redetermination requirements and improving application submission and processing timeliness.

Requirements

  • Ability to work independently and collaboratively in a remote team environment with limited in-person supervision.
  • Solid level of comfort with Microsoft Office Suite applications, including Microsoft Teams, Outlook, Word, and Excel.
  • Ability to gain in depth understanding of New Hampshire Medicaid long-term care financial eligibility criteria.
  • Ability to clearly explain application processes, documentation requirements, and complex eligibility rules to applicants with a diverse range of communication and functional skills.
  • Competency in applying quality customer service principles.
  • Strong organizational, documentation, and problem-solving skills.
  • Excellent communication and interpersonal skills, with sensitivity to diverse populations.
  • Bachelor’s degree in social work, human services, business administration, or a related field; or equivalent experience.
  • 1–3 years of experience working with individuals in long-term care, home and community-based services, community health, case management, or similar settings.
  • Ability to speak, read, and write English clearly and concisely.
  • Employment is contingent on completion of a background check.
  • Must have a private space with no distractions and reliable, high speed internet connection.
  • Must have reliable transportation to travel to onsite locations across the state of New Hampshire.

Nice To Haves

  • Familiarity with Federal and New Hampshire Medicaid financial eligibility regulations and processes preferred.

Responsibilities

  • Travel to on-site locations throughout New Hampshire, as assigned.
  • Assist individuals, families, and their representatives in completing NH’s Medicaid Application during scheduled in-person and virtual appointments
  • Guide applicants through completion of required forms (i.e. BFA-800), gathering acceptable documentation, and submission of their electronic application, while complying with State polices and procedures regarding system use and management
  • Explain income and asset documentation requirements to support financial eligibility determinations
  • Educate applicants about Medicaid long-term care financial eligibility requirements, including asset transfers, spend down, third-party liability and spousal impoverishment rules.
  • Review acceptable forms, documentation, and supporting materials checklists for application completeness prior to submission.
  • Provide regular updates to the Eligibility Supervisor and Project Management team regarding assistance delivered and barriers or process issues encountered.
  • Uphold State, Federal, and PCG confidentiality and security protocols and policies in all aspects of work.
  • Ensure professionalism and positivity when communicating complex process steps.
