About The Position

As we continue to grow, we’re expanding our field-based client services team - the heart of our client and caregiver experience. This role is ideal for someone who loves being out in the field, meeting families, supporting caregivers, and representing our agency in the community. You’ll conduct in-home assessments, maintain strong client relationships, and help drive growth through referral development and community engagement. It’s a hands-on, people-focused role - perfect for someone who thrives on connection, problem-solving, and making a meaningful difference every day.

Requirements

  • Experience in home care, home health, senior services, social work, or related fields
  • Strong communication and relationship-building skills
  • Comfort working independently in the field
  • A reliable, compassionate, service-oriented mindset
  • Ability to manage details while staying connected to the bigger picture

Responsibilities

  • Conduct in-home care assessments and reassessments
  • Build strong, trusting relationships with clients and families
  • Ensure care plans are accurate, personalized, and aligned with client needs
  • Serve as a responsive, compassionate point of contact for client concerns
  • Partner with caregivers to ensure successful client-caregiver matches
  • Provide field support, oversight, coaching, and communication as needed
  • Conduct annual reviews and supervisory visits
  • Help maintain continuity and quality of care
  • Represent With A Little Help at community events, senior centers, and partner organizations
  • Build and nurture referral relationships with healthcare and community partners
  • Identify opportunities to expand our presence and strengthen our reputation
  • Support marketing initiatives and local engagement efforts
  • Collaborate closely with the office team to ensure smooth scheduling and service delivery
  • Maintain accurate documentation and follow established processes
  • Support the overall client services workflow with reliability and professionalism
