About The Position

The Field RN Care Manager (Physical Health) is responsible for assessing and evaluating members' needs to help them achieve and maintain optimal wellness. This role involves guiding members and their families towards appropriate resources and facilitating interactions that support their care and wellbeing. The position requires a holistic, member-centric approach and involves both telephonic and face-to-face assessments to ensure members are progressing towards their health goals.

Requirements

  • Unrestricted Licensed Registered Nurse in the State of Louisiana.
  • 2 years of experience in nursing or healthcare field (discharge planning, case management, care coordination, and/or home/community health experience).
  • Extensive nursing experience across all ages.
  • Knowledge of community health and social service agencies.
  • Intermediate to advanced computer skills, including Microsoft Word, Outlook, and Excel.
  • Ability to use electronic medical records and various software applications.
  • Exceptional communication and interpersonal skills.
  • Ability to work with minimal supervision.
  • Ability to work a full-time flexible schedule.

Nice To Haves

  • Experience with telehealth services.
  • Bilingual skills.

Responsibilities

  • Assess and evaluate member's needs to achieve optimal wellness.
  • Employ strategies to manage a member's physical, environmental, and psycho-social health issues.
  • Identify and resolve barriers to effective care.
  • Engage and motivate members and their families through recovery and health programs.
  • Perform telephonic and face-to-face assessments of member needs.
  • Continuously monitor care through assessments and evaluations.
  • Develop care plans based on clinical judgment and changes in members' health.
  • Collaborate with providers and community services for quality outcomes.
  • Coordinate delivery of services for Physical Health and Social Determinants of Health.
  • Work across the transdisciplinary care team and manage transitions of care.
  • Submit incident reports as necessary.
