About The Position

POSITION SUMMARY: This position is responsible for directing the clinical operations of the companys non-medical home care services and personal care training in accordance with current federal and state guidelines in partnership with the Agency Director/Director of Operations. This is a management position, but does require a case load, in person admissions and supervisory visits to ensure client satisfaction and regulatory compliance. Hours: Full Time - Monday Through Friday - 8 AM to 5 PM Coverage Areas: Greater Cleveland and Akron/Fairlawn areas covering Cuyahoga and Summit Counties. Requirements

  • Bachelors degree in nursing required from an accredited college or university
  • Must possess a current, unencumbered, active license to practice as a Registered Nurse in the state of Ohio
  • Must have at least 2 years of experience in health care and supervisory/management experience.
  • Must possess a valid and unrestricted Driver's License, proof of insurance, and clean driving record

Responsibilities

  • Ensures the timely start of care within regulatory guidelines.
  • Performs accurate and complete admissions, re-admissions and safety evaluations.
  • Create customized non- medical care plans that give people the freedom to remain in the place where they call home.
  • Perform routine supervisory visits to evaluate the plan of care and supervise delivery of excellent client service.
  • Develop and maintain a system for supervisor visits for PRN clinical team.
  • Present educational in-service training.
  • Evaluate and ensure that competency training goals are met for non-licensed care professionals.
  • Ensures training goals are met for all new hires and annual in-services under the direction of the corporate quality department.
  • Coordinate/oversee/supervise the work of aides during Supervisory Visits.
  • Communicate with Case Managers and other members of the care team.
  • Develops, initiates and revises Plan of care within a person-centered framework.
  • Always maintains a high degree of confidentiality due to access to sensitive information.
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department.
  • Follows all Medicaid, and HIPAA regulations and requirements.
  • Abides by all regulations, policies, procedures and standards.
  • Conduct chart reviews weekly to ensure ongoing compliance with state regulations.
  • Assist with incident reporting (reviewing and follow-ups to ensure positive resolution).
  • Performs on-call responsibilities and provides on-call services as needed.
  • Remain available for field work as needed (back up only for operational on call team for emergencies only).
  • Perform other duties as assigned.
