Requirements

• Active Texas RN license
• Available 36–40 hrs/week
• Able to travel within assigned counties
• Comfortable in home health settings

Nice To Haves

• Kantime EMR or EVV experience
• Prior home health experience
• Bilingual (Spanish)

Responsibilities

• Deliver skilled nursing care in patient homes
• Follow defined care plans and clinical protocols
• Monitor vitals, meds, and home medical equipment
• Chart visits in Kantime EMR (simple, modern)
• Coordinate with families and clinical team
• Support safe transitions between care settings
