About The Position

The Field Nurse is responsible for both providing direct nursing supervision for clients and HHAs and serving as the lead educator for the HHA certification program. This dual role assists the Clinical Manager in ensuring quality client care in the home setting while also preparing and mentoring future HHAs through classroom instruction, skills training and clinical supervision. The position supports the agency’s mission by maintaining high standards of care, compliance with state training requirements and ongoing professional development for HHAs.

Requirements

  • Active Registered Nurse (RN) license in good standing
  • Minimum of 2 years of nursing experience, with experience preferred in home care, long-term care, or community health
  • Experience in teaching, training, or supervising aides strongly preferred
  • Knowledge of state requirements for HHA certification programs preferred
  • Knowledge of Licensed Home Care Services Agency (LHCSA) state regulations preferred
  • Excellent teaching, leadership, and interpersonal skills
  • Strong knowledge base on clinical aspects of home care, including infection control and chronic disease management
  • Reliable transportation and ability to travel to client homes and training sites throughout agency territory

Responsibilities

  • Deliver classroom, skills lab and clinical instruction in accordance with state regulations and approved curriculum.
  • Teach core HHA topics including personal care, infection control, client safety, communication, nutrition, vital signs, mobility support and client rights.
  • Demonstrate and supervise return demonstrations for skills such as bathing, transfers, ambulation, feeding and vital sign monitoring.
  • Evaluate student progress using written exams, skills checklists and direct observation.
  • Provide constructive feedback and individualized support to meet the needs of adult learners.
  • Maintain accurate records of attendance, progress and competency completion.
  • Foster a professional and supportive learning environment that promotes respect, compassion and high-quality care.
  • Collaborate with administrators to update training materials, lesson plans, and competency tools.
  • Provide nursing supervision to clients in their homes according to physician orders and agency policies.
  • Conduct client assessments, develop and update care plans and monitor health status.
  • Supervise and support HHAs in the field to ensure care plans are followed and best practices are upheld.
  • Conduct supervisory visits to evaluate aide performance and client satisfaction.
  • Serve as a clinical resource for aides, offering on-the-job guidance and reinforcement of training.
  • Communicate effectively with clients, families, HHAs, office staff and the interdisciplinary care team.
  • Document care and supervisory activities in compliance with agency policies and regulatory standards.
  • Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Dignity standards are always met.
  • Perform other duties as assigned.
