About The Position

In this role, the successful candidate Provides safe, quality, therapeutic age appropriate nursing care using evidence based practice. Uses clinical expertise in assessment, clinical decision making, clinical practice and patient education. The nurse coordinates the care provided with the interdisciplinary team to promote optimum patient outcomes and satisfaction.

Requirements

Current Registered Professional Nurse in New York
Bachelor's degree in Nursing required
Basic Life Support Certification through the American Heart Association
Fluent written and spoken English language skills
1- 2 years Acute care facility OR Home Health,Hospice, and Long Term Home Health Care Program
Excellent communication (both oral and written), interpersonal, customer service skills
Critical thinking skills
Working knowledge of Computers.
Registered Nurse License-NYS
Basic Life Support Cert

Responsibilities

Performs other duties as assigned
Completes a comprehensive nursing assessment including but not limited to physical, functional, psychosocial, emotional, cognitive, sexual, cultural, age related, environmental, spiritual, transpersonal and economic factors in a systematic and ongoing process while honoring the uniqueness of the patient.
Derives the diagnoses and needs from the assessment data, validates the diagnoses, and collaborates with the patient, family, caregivers and physician to develop an individualized plan care to attain expected quality outcomes.
Responsible for the coordination of care by interacting and collaborating with other members of the healthcare team to optimize patient outcomes, patient satisfaction and health care goals.
Delivers direct nursing care, ongoing assessment, teaching and training to patientscaregivers that preserves and protects patient autonomy, dignity, rights, values, and benefits using evidence based nursing practice.
Identifies deficits and barriers to effective knowledgeunderstanding of education provided to patients. Employs strategies to optimize patient engagement. Demonstrates caring behaviors toward patients, family and other caregivers.
Complete and maintain accurate, complete and timely documentation reflecting the assessment, examination, diagnosis, prognosis, plan of care, education intervention and evaluation according to Federal, Department of Health and The Joint Commission requirements.
Assist Agency leadership with achieving corporate goals, improving quality of patient care outcomes and patient satisfaction. Participate in Agency quality assessmentperformance improvement initiatives.
