Requirements

Licensed Practice Nurse license certification in the state of employment
Ability to conduct independent home visits
Experience providing care for the whole person, including caring for adults and seniors with high medical and social complexity.
Current CPR and BLS certification required
Average to advanced computer & software skills (Patient Health Exchange Platforms, Electronic Health Records, and Outlook)
Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years. No more than 2 moving violations or 1 at-fault accident in the last 3 years.

Responsibilities

Partner with care team members to develop and implement patient-centered clinical plans.
Deliver wrap-around services through home visits, and collaborative care alongside providers and RNs.
Perform key clinical tasks in the field— chronic condition support, wound care, phlebotomy, medication review, and more.
Engage hard-to-reach patients, bringing them vital care and support directly at home.
Ensure continuity of care by providing coverage for team members when needed.
Help drive better patient outcomes, lower costs, and increased patient and staff satisfaction through our interdisciplinary care model.
