About The Position

ConcertoCare is seeking a dedicated Field Nurse (LPN) to redefine healthcare for seniors and adults with complex needs. This role involves delivering holistic, human-first care directly in patients' homes, blending compassion, technology, and innovation. Requirements

  • Licensed Practice Nurse license certification in the state of employment
  • Ability to conduct independent home visits
  • Experience providing care for the whole person, including caring for adults and seniors with high medical and social complexity.
  • Current CPR and BLS certification required
  • Average to advanced computer & software skills (Patient Health Exchange Platforms, Electronic Health Records, and Outlook)
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
  • DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years. No more than 2 moving violations or 1 at-fault accident in the last 3 years.

Responsibilities

  • Partner with care team members to develop and implement patient-centered clinical plans.
  • Deliver wrap-around services through home visits, and collaborative care alongside providers and RNs.
  • Perform key clinical tasks in the field— chronic condition support, wound care, phlebotomy, medication review, and more.
  • Engage hard-to-reach patients, bringing them vital care and support directly at home.
  • Ensure continuity of care by providing coverage for team members when needed.
  • Help drive better patient outcomes, lower costs, and increased patient and staff satisfaction through our interdisciplinary care model.
