Assisting Hands (Hudson) is looking for an experienced RN with a valid license in New Jersey and a minimum of 1 year of Home Care experience.
Responsibilities
- Conducting thorough client assessments and re-assessments
- Formulating comprehensive care plans
- Performing 30-day check-in and 60-day care monitoring visits
- Conducting supervisory visits
- Regular communication with patients, families, caregivers and care-stakeholders
- Training Caregivers
Requirements
- Valid Nursing license in State of New Jersey
- At least 1 year experience in Home Care is preferred
- Has a valid driver's license and a reliable mode of transportation
- Able to travel to clients’ homes throughout Hudson and South Bergen county
- Strong communication and interpersonal skills
- Professional Liability Insurance 1M – 3M
