About The Position

The Field Nurse Practitioner role at Advantmed involves conducting in-home wellness risk adjustment assessments for Medicare members and other at-risk populations. The primary objective is to assess the overall health and well-being of beneficiaries to ensure accurate risk adjustment coding, thereby enhancing value-based care. This position offers flexibility as it operates on a PRN (1099) basis, allowing practitioners to balance competing priorities while providing quality care.

Requirements

• Must have a valid unencumbered NP License for the state of Florida.
• Access to reliable transportation for travel within a 55-mile radius.
• Previous in-home risk assessment experience preferred.
• 3 years of patient care experience preferred (primary care/adult/geriatric, EMR).
• Strong ability to work within an EMR system.
• Ability to work independently.
• Bilingual is a plus.

Nice To Haves

• Previous 1099 (PRN) experience is preferred but not required.

Responsibilities

• Perform annual in-home wellness visits and risk adjustment health assessments on a population with chronic conditions.
• Conduct comprehensive assessments including physical examinations, vital signs, medication history reviews, and cognitive assessments.
• Deliver patient health education on topics such as pain management and medication.
• Build relationships of trust with members through exceptional communication and empathy.
• Assist in closing quality care gaps, including screenings and labs.
• Collaborate enthusiastically to enhance care delivery.
