Requirements
- Active LPN or RN license in Virginia or valid multi-state compact license
- Home health or home care experience preferred
- Skilled in trach, vent, and G-tube care
- Reliable, flexible, and able to work independently
Nice To Haves
- Home health or home care experience preferred
Responsibilities
- Provide direct patient care following the plan of care
- Monitor and manage trach, vent, and G-tube care, including emergencies
- Complete all clinical documentation accurately and on time
- Perform skilled nursing services as needed
