About The Position
ConcertoCare is seeking a Licensed Practical Nurse (LPN) to be a core member of a multi-disciplinary Field Team that delivers home- and community-based care to patients with complex needs. Our mission is to provide seniors and older adults with complex care needs a more holistic, equitable, and compassionate approach to health and wellness. The LPN will deliver care through co-visits with other team members, assist with telehealth visits, and conduct independent LPN visits. Responsibilities include conducting health screenings, performing clinical services, and providing clinical support by partnering with interdisciplinary team members.
