About The Position
ConcertoCare is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to be a core member of a multi-disciplinary Field Team that delivers home- and community-based care to patients with complex needs. Our mission is to provide seniors and older adults with complex care needs a more holistic, equitable, and compassionate approach to health and wellness. The role involves delivering care through co-visits with other team members, assisting with telehealth visits, and conducting health screenings for newly enrolled patients.
