Field Nurse Case Manager - Saginaw, MI
About The Position
Paradigm is looking for a full-time, 0.75 - 1.0 FTE (30-40 hours per week), benefited Field Nurse Case Manager to cover the Saginaw and Bay City, MI, areas. This is a salaried position: base salary range at 0.75 FTE is $68,000-$73,000. At 1.0 FTE, base salary range is from $88,000-$93,000 annually. A Field Case Manager role is a home-based position with travel, up to 2 hours one-way (4 hours round trip), to doctors’ offices, hospitals, and various other locations. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers.
Requirements
- Current, unencumbered Registered Nurse (RN) license in Michigan required
- Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal)
- Ability to operate autonomously with minimal oversight
- Skilled at patient education
- Valid driver’s license and good driving history
Nice To Haves
- Bilingual (Spanish speaking) strongly preferred
- Experience in case management of workers comp cases strongly preferred
- Recommended certifications (CCM, CRRN, CDMS, CRC) strongly preferred (must obtain within two years of hire date)
