About the position

The Field Manager at Work By Faith Home Care is responsible for overseeing the delivery of personal care services to clients, ensuring compliance with state regulations and client care plans. This part-time role involves supervising caregivers, conducting home assessments, and maintaining communication with clients and office staff to ensure high-quality care and service continuity.

Responsibilities

  • Complete initial evaluations and home assessments for clients.
  • Monitor hospitalizations and service interruptions.
  • Assess and confirm service plan tasks meet clients' needs.
  • Ensure coordination of care with required forms and communication.
  • Schedule services according to service plans and authorized hours.
  • Conduct training with caregivers and ensure compliance with client requirements.
  • Conduct client orientations to ensure adherence to care plans.
  • Supervise caregivers to meet individualized service care plans.
  • Provide training on Electronic Visit Verification (EVV) procedures.
  • Assist with servicing clients to avoid service interruptions.
  • Maintain daily contact with office support staff.
  • Report concerns or changes in clients' conditions to appropriate agency staff.
  • Perform personal home care supervisory visits.
  • Orient caregivers to new assignments and clients' needs.
  • Determine caregivers' competency to perform tasks.
  • Resolve problems between clients and caregivers.
  • Assist with evaluations of caregivers' job performance.
  • Complete required documentation for supervisory visits.

Requirements

  • Two years of college or equivalent supervisory experience in a healthcare facility, home care agency, or community agency.
  • Bachelor's degree preferred.
  • Valid driver's license and auto insurance in accordance with state laws.
  • Excellent customer service skills.
  • Ability to cope with elderly, ill, and disabled clients respectfully and professionally.
  • Punctual and reliable with a dependable vehicle.

  • Knowledge of general office terminology, procedures, and equipment.
  • Ability to work independently and exercise good judgment.
  • Ability to prioritize workload and organize time effectively.
  • Experience working with community organizations and resources.
