About The Position

Requirements

• Active, unrestricted LVN/LPN license for the state of Texas.
• 2+ years of clinical experience in a health care role.
• Intermediate Microsoft Office experience, including Word, Excel, and Outlook.
• High-speed internet access in residence.
• Reliable transportation and ability to travel within the assigned region.

Responsibilities

• Assess, plan and implement individualized care strategies for patients.
• Identify and initiate referrals for social service programs, including financial and community support services.
• Manage the care plan as a single point of contact throughout the continuum of care.
• Communicate health-related information to all stakeholders to ensure quality coordinated care.
• Advocate for patients and families to ensure their needs and choices are represented and supported.
