About The Position

The Field HSS Clinical Coordinator RN role at UnitedHealth Group involves assessing, planning, and implementing individualized care strategies for patients in the Bellaire, Missouri City, and Stafford, TX areas. This position is primarily field-based, requiring significant interaction with patients in their homes or long-term care facilities, while also collaborating with healthcare providers to ensure quality care and support for patients and their families.

Requirements

  • Current unrestricted RN license in the state of Texas.
  • 2+ years of experience in a community health setting or healthcare role.
  • Intermediate experience with Microsoft Office (Word, Excel, Outlook).
  • Reliable transportation and ability to travel to visit Medicaid members.
  • High-speed internet at residence.

Nice To Haves

  • 1+ years of experience with long-term care services and support, Medicaid or Medicare.
  • Ability to create, edit, save and send documents, spreadsheets, and emails.
  • Knowledge of federal and state requirements for integrated settings.
  • Reside within commutable distance of assigned duties.

Responsibilities

  • Assess, plan and implement individualized care strategies for patients.
  • Utilize company and community resources to establish effective case management plans.
  • Collaborate with patients, families, and healthcare providers to develop care plans.
  • Identify and initiate referrals for social service programs.
  • Manage care plans throughout the continuum of care as a single point of contact.
  • Communicate health-related information to stakeholders for coordinated care.
  • Advocate for patients and families to ensure their needs are represented.
  • Utilize clinical criteria to assess appropriate levels of care.
  • Document member assessments, care plans, and referrals.
  • Participate in interdisciplinary team meetings and utilization management rounds.
