About The Position

The Field Community Health Worker (CHW) at UnitedHealth Group will serve as the primary care manager for members with complex medical or behavioral needs, focusing on those not managed by the clinical team. This role involves engaging with members in the community, developing individualized care plans, and addressing barriers to health equity. The CHW will collaborate with an interdisciplinary team to implement interventions that align with members' readiness for change, ultimately promoting health equity and improving health outcomes.

Requirements

  • High School Diploma / GED (or higher)
  • 3+ years of experience in a Community Health setting
  • 2+ years of experience working within the local community
  • Intermediate knowledge of Medicare and Medicaid benefits
  • Intermediate proficiency with Microsoft applications
  • Access to reliable transportation for travel to client sites
  • Ability to travel 25% of the time
  • Access to a designated quiet workspace at home with high-speed internet

Nice To Haves

  • Bachelor's Degree in Social Work or related subject
  • LPN Licensure or CNA/HHA
  • Bilingual in Spanish
  • Community Health Worker Accreditation
  • Experience in team-based care
  • Experience in Managed Care
  • Lived experience in managing chronic health conditions

Responsibilities

  • Partner with clinical care management team to manage complex members in the community
  • Locate and engage members in the community
  • Conduct screenings face to face or telephonically to assess member needs
  • Research, locate and make referrals to community resources for member needs
  • Perform targeted activities to support HEDIS/STAR gap closure
  • Proactively engage members to manage their care and promote self-management
  • Collaborate with internal care team, community partners, and providers
  • Consult with clinical team based on defined triggers
  • Support identification and coordination of Medicaid benefits
  • Assist members in developing personal wellness and health crisis plans
